2016-2017 Sports Physicals

Time to start planning for pre-participation physicals to carry your 

student-athletes through the 2016-2017 school year.

 Physical's must be dated after April 1st, 2016 in order to be valid for the 2016-2017 School Sports Season.

With that said, Richland 2 is once again partnering up with 

Palmetto Health Orthopedics planning for pre-participation physicals to carry your student-athlete through the 2016-2017 school year.

Palmetto Health Orthopedics will be hosting our annual mass physical day 

on Saturday April 9, 2016 at 10:00 am, for middle school students, at our Lexington Office located at 104 Saluda Pointe Drive.

Physicals are $10.00.
Checks can be made out to Blythewood Middle or you may pay online through parent portal under student fees.

Any Student interested MUST register with Mr. Ratterree, and submit payment,  by Wednesday, April 6th, 2016.

All forms must be completed in their entirety prior to arrival, including medical history and signatures, or no physical will be given. It is important that the Parent Permission and Acknowledgement Of Risk form is signed and dated.  

Make-up physicals will not be offered by Palmetto Health Orthopedics. If this opportunity is not seized, it will be the responsibility of the student-athlete and parent/guardian to obtain a physical prior to their team(s’) first practice in order to participate. 

It is important to note we will not be offering cardiology exam and clearance at this year’s Physical Day. Anyone needing additional cardiac evaluation will need to follow up with their primary care physicians for clearance.

All athletes are urged to wear comfortable sports attire (ie: shorts, t-shirts, tennis shoes, etc…) that will allow the individual to perform basic movements required during the physical examination.
